200 Club Fall Tournament Team Sign Up Form

• Please fill out form for entire team, please just one form per team. If you are a single or don't have a full team please fill out and submit. 

 

• Afternoon shotgun will be limited to the first 23 teams that sign up. If you're interested, you can participate in the morning shotgun with limited spots available. Any team that signs up after the first 23 afternoon groups will have to play in the morning.

 

• If you're planning on eating dinner at Terrazza after the event, reservations are required! Thank you in advance. Call 413-772-0970 to do so.

 

• Mulligans and an entry into the raffle are available as well. The cost is $20 per person and should be purchased by CLICKING HERE
